Start by completing Form 4A (Original Owner(s) Claim) and Form 5 (Indemnity Agreement). Ensure both forms are duly commissioned by a lawyer.
Secure an original official letter from the holder (e.g., bank or insurance company) confirming the remittance of your unclaimed financial assets to the UFAA.
Provide a certified copy of your National Identity Card or Passport, certified by a lawyer. Also, include a copy of your KRA PIN certificate.
Indicate your payment details on the Payment Details form. Ensure the information is accurate and matches your bank or M-Pesa account.
